Instructions

  1. Complete application portions of applications.
  2. Attach 2 workbooks; original forms including WS1, WS2, or WS3 and WS4. Right click and choose 'save as' on the link to download the excel file.
  3. Include all required file attachments as pdfs in one compressed zip file (*.zip). All other file types will not be accepted. Maximum total file size is 2MB.
  4. Complete the required payment information.
  5. To submit an initial application via postal delivery, submit the following:
    a) Printed and completed application form.
    b) One hard copy and four disks with electronic files OR five hard copies of the supporting documentation.

I hereby state that the approved Module I workshop and/or Module II Seminar will be conducted as described in the application and that the Certificate Board may survey audit our course (s) at any time, at no cost to us. I retain the right to refuse individual on site auditors where a client/confidential or competitive situation exists.

I realize that this approval is only good for four (4) years and must be renewed under the requirements in force at that time.

Note: The prime CVS is responsible and accountable to the Certification Board for the conduct and performance of the seminar/workshop. All affililiates are accountable to the prime CVS.
